24 of 24 · most liked firstSuch a fun game concept, and man is it addicting. But to dump 40 hours in on a few different saves trying to progress I've found this debilitating bug. After you've built up the first few departments if you try to add anything else it unassigns all nightshift staff from their workplaces. No biggie, just reassign them right? Nope that slot then become unusable forcing you move and place back all the PCs. I looked online and sure enough people have been talking about this bug for 6 years. For…

I'd be more interested if the campaign started off in the 3rd world with very little equipment, it would demonstrate how improving medical equipment improves survival rates.
negative · reviewer's own thumbsView post →Project Hospital is crunchy management and deep simulation steeped in real-world medicine. I don't know how to explain my absolute infatuation with this except to say that after 65+ hours, it feels like I've only scratched the surface of the base game. Gameplay + Details At first glance, this looks like average top-down management: build hospitals one room at a time, hire employees, take loans, balance budgets. Then a patient dies and the game asks, "Would a more efficient hospital be able…
positive · reviewer's own thumbsView post →Great game, wish i could find any other games like this. I would love to see more departments and diffrent types of staffing.
positive · reviewer's own thumbsView post →Please make a sequel to this. Easily a top 5 realistic game for me. If you ever wondered why hospitals are so complicated play this game and it will show you.
positive · reviewer's own thumbsView post →Fun game but I have a couple of gripes. Firstly I wish the undo feature went back further than one action. Secondly I wish there was more to actually running the hospital. The bulk of the game is building with a little bit of diagnosis on the side. Otherwise though this feels like a grown up version of Two Point Hospital which is what I was looking for.
positive · reviewer's own thumbsView post →I wake up every day resentful that his masterpiece is not getting another dlc or sequal.
positive · reviewer's own thumbsView post →This game is hands down one of the best simulators I've ever played, I truly with they would make a Project hospital 2. It deserves an overwhelmingly positive!
positive · reviewer's own thumbsView post →I keep trying to play this game because I really want to like it because I love a hospital or management game but I just could not get locked in. I think for me the issue is that while in a lot of ways the UI is great and I like staffing and building a rotation, etc., that I found it really difficult in the area of knowing what to build and how to do it and there's really not tutorial aspect that provides help with that.
negative · reviewer's own thumbsView post →This is a tough game to rate and therefore hard to say if I recommend it or not. Project Hospital is a sandbox management game where you build and manage your own hospital. It is very similar to Two Point Hospital, just more realistic and way more “serious”. This game goes really hard in on management and less so on being a creative sandbox. Patients have to go through every step of the process and mishaps often lead to injuries and death. Hospital employees all have their own employee cards…

There are some limited machinery, appliances, furniture, and features. btw I like the game so much, but what i hated the most is lack of furniture, i am expecting more I want moreeeeeeeeeee!!!
positive · reviewer's own thumbsView post →I absolutely LOVE this game, I just wish they would add new content in the updates. It can get slightly repetitive after a while but I still completely love it. A police version would be cool with functional prison, jail, detectives with investigation offices, Pat down, and forensics. Events such as raids, Prisoner riots etc. Depth could be from simple petty crimes where criminals are brought in and serious crimes where they're sent straight to jail/ questioning. Just an idea, but id LOVE…
positive · reviewer's own thumbsView post →I have put almost 200 hours into the game and still feel that I have not scratched the surface. For context, I only played the tutorials and then went straight to building my personal hospital in the sandbox mode. I'm currently building my third hospital after learning from the mistakes I made in the first two. I also use many of the amazing mods made by the community that add so much extra customization, departments, diseases, etc. The fact that Oxymoron games made it so easy to modify the…
positive · reviewer's own thumbsView post →One of my favorite games and has super high replay value. The details and gameplay is simple to pick up. I've played the campaign and sandbox mode multiple times. Modders added additional sections and Depts to the hospital. At the time of this review, I have more than 100 hrs and plan to gain more. Hoping this game with see a good sequel or add on to the current version, although unlikely hoping for the best.
positive · reviewer's own thumbsView post →This game is everything I've ever wanted from a management sim - floorplans, staff management, and my favorite part DOCTOR MODE! If you've ever wanted to pretend to be Meredith Grey, this is the game for you, you get to actually decide which tests to run and diagnose the patients. My only complaints are that when you first start the tutorial doesn't thoroughly explain everything, even almost 200 hours in I am still learning news tips and tricks from Reddit or through trial and error. I prefer…
positive · reviewer's own thumbsView post →I love building hospitals that are chaotic but functional. I found this game from Let's Game It Out and have been inspired to build some... open concept hospitals with an empasis on community and together. Every room, including isolation has a well used vending machine.
positive · reviewer's own thumbsView post →If you enjoy pathway management, bottleneck analysis, and layered systems — this game delivers. What hooked me: Real diagnostic chains (you can’t just guess, you need proper tests) Staff specialization actually matters Insurance system influences patient flow Department layout directly affects efficiency Emergency overloads expose weak design instantly You don’t just build rooms. You design flow architecture. If radiology is too far from emergency? Delays cascade. If your night shift…
positive · reviewer's own thumbsView post →Easily the best game I have ever played. I'm so sad they don't do updates anymore, but honestly modding makes up for it. Very realistic, amazing game to play for people with a medical background or people wanting to learn more about the medical field!
positive · reviewer's own thumbsView post →Janitors are very great. They can't let operating room dirty even when there is an ongoing operation. 11/10

unlabelledView post →I hope I am being respectful while sharing my honest thoughts. While the game is potentially fun and very detailed, it was only fun for me for a couple of hours, and 1-2 of which was learning the ropes in the game (it doesn't help that the tutorial was not at all straightforward and it didn't really give me enough information to be able to progress. I had to search on YT how other players had done it). After the tutorial, the game had some "campaigns" set up that didn't really ease you into…
negative · reviewer's own thumbsView post →I like it, come back to it periodically. I don't care about the hospital building aspects. I like the diagnosis and treatment aspects.
positive · reviewer's own thumbsView post →Quite buggy, the tutorial isn't great and combine it with bugs and it breaks. I wasn't expecting Rimworld in Hospital form, to be honest. Everything is incredibly tiny and scaling the UI to a comfortable place sets things off center for the menu. I'm not sure how this game reached Overwhelmingly Positive, but clearly it wasn't meant for me. Haha
negative · reviewer's own thumbsView post →The Short: Great management game with potential to sink hours if not days into. Great modding community that provides extra content to give a try. The Long: (Warning I'm going to glaze this game) Lets begin at the main menu, the soundtrack that plays right off the bat is already a jock jam, the whole album is well made. But I think the composer for the main menu song does the game justice. Giving a slow almost clinical rhythm, slowly escalating as the sound of helicopter blades and ambulance…
positive · reviewer's own thumbsView post →The UI for this game is not the best and not user friendly. Does remind me of prison architect in a way where you get a great simulation game concept but ruin it with unnecessary building requirements. Or building requirements that take a lot of fun away from the game. Possibly good for people who play the sims but would rather build houses than on live mode. I did not get far in the game because of the tiny font and poor interface. I am aware there are settings to make font 'bigger' but even…
negative · reviewer's own thumbsView post →The greatest hospital game ever made. Truly spectacular. I love pretty much everything about it from building my own hospital, designing wards and rooms to treating the patients. I have all the DLCs and I must say they are great and mods make the whole experience even better. What I didn't expect was to see a tutorial which really helped. So you don't have to worry about needing to be a hospital architect or doctor. I've played for quite a while and yet I still have a long way ahead of me…
positive · reviewer's own thumbsView post →I've never had a game make me angry. It has the absolute worst tutorial. You've gotta go to youtube in order to figure out how to buy staff. then when you figure it all out, it turns out you did it all wrong and you need to do it all over again and assign everything based on departments. Piss poor UI
negative · reviewer's own thumbsView post →It's an okay game. The campaigne throws you at fully loaded hospitals that only need a little tuning. You don't build too much on your own. You never start from scratch, it's just opimizing what is already there. The idea is good, but it's nothing compared to like Two Point Hospital where you really have different scenarios. Most of the time you are just waiting after optimizing the hospital. Not much to do.
negative · reviewer's own thumbsView post →I love how this game is ACTUALLY interactive - from being able to treat patients yourself, managing staff & building a hospital from scratch, it really allows you to use your own creativity. When you start out it can be a bit tricky to learn the ins and outs of the mechanics, but once you get there, the game becomes fun and almost satisfying to play! My only critique is that I wish that there were more DLC, including a maternity or children's ward. I would definitely recommend this game to…
positive · reviewer's own thumbsView post →Oddly addicting. Especially when you figure out how to change code. It is written in HTML so it is really easy.
positive · reviewer's own thumbsView post →Not recommended—at least, it wasn’t for me. Project Hospital felt too focused on detailed management and administration, without enough creative freedom or enjoyment to keep me interested. I spent more time managing systems than actually having fun building and developing my hospital. Maybe I was playing it the wrong way, and people who enjoy deep management simulations may love it, but I couldn’t get into it.
negative · reviewer's own thumbsView post →Project Hospital is genuinely a great game, and I’d go as far as saying it’s the best game in its genre. I can easily lose hours playing without even realizing it. It feels like a true hospital management simulator rather than a simplified management game. There are so many components that other hospital sims just don’t have, especially the ability to take control of individual patients and actually manage their diagnosis and treatment. That adds a whole other level of depth and makes the…
positive · reviewer's own thumbsView post →omfg the UI is kind of unbearable. There are so many weird softlocks you can get yourself into and I had to restart the tutorials multiple times to get through them.
negative · reviewer's own thumbsView post →Project Hospital has been an incredibly rewarding experience. With over 400 hours played, it’s a game that consistently proves how deep and engaging it can be as well as frustrating. The level of detail in hospital management, diagnostics, and patient care is impressive, and it strikes a great balance between realism and fun. Whether designing departments, managing staff, or handling complex cases, the gameplay remains challenging and satisfying even after hundreds of hours.
positive · reviewer's own thumbsView post →I think this game is great especially if you like management styles of games. I like it a lot bc its very busy and as someone who has ADHD a game like this where theres so much going on, I find really good at helping me escape. All in all great game and I cant wait to play the DLCs once I get them
positive · reviewer's own thumbsView post →Underrated game. All of the important parts are super fleshed out. High level of replayability.
positive · reviewer's own thumbsView post →Project Hospital Made Me Learn Real Medicine By Almost Killing a Guy With Lead Poisoning Project Hospital lets you run things two ways. At the micro level, you take control of an individual doctor (you can even rename yourself into the role) and work clinic patients directly, running the actual exams (physicals, chest exams, skin allergy tests, and so on) to reach a diagnosis. At the macro level, you're managing the whole hospital instead: expenses, patient flow, staffing, the works. Both…
positive · reviewer's own thumbsView post →After more than 440 hours in Project Hospital, I can say the game has delivered one of the most consistently engaging management experiences I’ve played. I completed all challenges (including DLC) and earned every achievement, all in mostly vanilla playthrough with just one mod (Bedside Examinations). The progression felt smooth, the systems remained rewarding even deep into late‑game, and the level of control over hospital operations kept me invested throughout. With the core game fully…
positive · reviewer's own thumbsView post →Incredible and very realistic game! My mother - who is a qualified nurse - played this game and said it was so realistic that she couldn't believe it!
positive · reviewer's own thumbsView post →I go through phases where I still come back to this game. I like planning hospitals for some odd reason.
positive · reviewer's own thumbsView post →This is, in my opinion, not just the best medical game, this is the best management game I've played. You can manage all the way down to one on one patient care as a doctor, manage an entire department, or manage the entire hospital. It's overwhelming at first but as you learn how the game works and how patients respond to different things, the game becomes intuitive. I'm 15 hours in and just now starting to get a feel for the game and how I should proceed. I recommend playing through the first…
positive · reviewer's own thumbsView post →We have just released a new patch for Project Hospital on Steam and GOG! In the meantime, the game is taking part in the #FlashGameVibes event on Steam, so you can get it with all the DLCs with a nice discount! Release notes: store.steampowered.com/news/app/868...
unlabelledView post →great game but i think. you need at least 1 of the dlc to make the game fun so it cost a bit more than the base game but i love that it teaches you a little bit of medical stuff
positive · reviewer's own thumbsView post →The barrier to understand this game is truly steep, but once it clicks it is madly addictive. Love unlocking achievement easter eggs that relate to hospital pop culture.
positive · reviewer's own thumbsView post →Amazing game, would reccomend the collection! DLC's are great, and I can tell they researched real medical conditions.
positive · reviewer's own thumbsView post →Silence of the Siren is a new HoMM-like from the dev of Project Hospital
unlabelledView post →The best hospital/medical simulator out there, very in depth and the doctor DLC is amazing and lets you be in charge of diagnosis.
positive · reviewer's own thumbsView post →Great experience playing this sim, highly recommended. Picked it up a few days ago and am already close to 24 hours in.
positive · reviewer's own thumbsView post →Love it. Lots of different ways to play. No playthrough seems the same. Only limited by your imagination.
positive · reviewer's own thumbsView post →Takes time to understand the game but super addicting playing doctor
positive · reviewer's own thumbsView post →It's a nice, cosy game- not too complex, but not a walk in the park either. The fundamental challenge is to get your hospital going, expand it slowly so that it may start earning on itself. Once you have a full clinic and 2-3 hospital wards you just snowball provided you do not expand too quickly. Occasionally you challenge yourself by initiating events that typically brings 50-100% more patients and test whether you can hold. In my 50 hrs I did not notice any issues.
positive · reviewer's own thumbsView post →The absolute worse UI/tooltip system I've ever encountered - trying to place a diagnostic room for surgery department, place all items in the room.... missing equipment apparently, would not register the patient exam bed thing whatsoever Absolute crap - needlessly overcomplicated
negative · reviewer's own thumbsView post →Confusing game. Looks like fun, but actually is very non-intuitive UI which is hard to digest. Tried going through tutorial, but failed. I guess you could get used to it, but it is hard and not fun for me.
negative · reviewer's own thumbsView post →Really enjoy this game. Thanks to the person who suggested it if you like Sims, Rimwworld, and Project Highrise. I love all three of those games, and I am loving this too. My only qualm is that the build mechanics can be tricky - it's hard to edit something you already made, and to paint the walls. Regardless it's a fun game. Can't believe I never found this before.
positive · reviewer's own thumbsView post →I'll make it simple: Do you enjoy the SIMS? Do you like Scenario-driven basic business management games like any good Zoo or Theme park game? Do you like managing sims and building in general like RimWorld, Project Highrise, or Prison Architect? If you say YES at all then you need to give it a try. This is The best hospital SIM i've ever played, alongside Two Point.
positive · reviewer's own thumbsView post →might be one of the best if not the greatest hospital management game ive ever played followed by two point hospital
positive · reviewer's own thumbsView post →So much depth, so much freedom, while not overwhelming you and keeping it in bite size digestible chunks. I simply adore the game, one where you can very easily lose track of time while playing. Would love to see the devs tackle a similar genre but as something outside of hospitals. They've made gold here!
positive · reviewer's own thumbsView post →Honestly, hits the spot for those who love the classic tycoons and Software Inc. This game is all my time after finally getting it on sale. -Still wish they had security. Can't think of a hospital without guards police or both. The world we live in.
