All of the extra stages and bosses from Returns are absent and the bad guy is the original head changing wizard instead of the devil that was introduced in Returns. Though I'm more familiar with the NES Hudson's Adventure Island version of the original game, this version is outstanding due to the enhanced visuals, clean and efficient UI, and improved gameplay. Definitely give it a try. Also, I'd love it if the gamemaker decided to offer an alternative version using Hudson's Adventure Island's main character and music.

The momentum of the jump and run are all off. The night stage does not have the "scary" music. Many of the stages are much easier (i.e 7-2 cloud jump). Yes, the game scrolls at 50Hz! 50Hz!!! It causes horrible and unnacceptable choppy screens while you play, leaving you with headaches after some time trying it. Simply infuriating!!!! But this introduces shimmering since the game seems to run at 50Hz. It is headache inducing and almost makes it unplayable. Not that you'd be missing much sine there really isn't much to the game at all. Just go play the original.

This is a great little remixed remake with smartly applied QoL that keeps most everything but cuts through some of the quarter muncher save-state addled tedium. Really good game for the Steam Deck, as well. Colors absolutely explode off of the OLED like it's some sort of tech demo. You can run it at 5 Watts @ 90 FPS, as well.
